1. This page is for Philippine and International Subscriber and Donor. Both of them can use the  applicable Form. All Forms has a Dropdown Menu that one can select anything of his choice. Please choose appropriate category.

2. For payment using Paypal, please pay first using paypal then fill up the Form For Paypal Payment so that your subscription or donation will be properly recorded.

3.  You can use the Form if you have pay through Philippine and international Remittance Centers, Bank and Paypal. Remember that you can still pay through Paypal even you have no Paypal Account. In payment page, choose the option to through your Debit/CreditCard.

4. Every Submission of the Form NEEDS A PHOTO/SCAN COPY OF 'PROOF OF PAYMENTS' like Official Receipt ( If payment made through Remittance Center), Deposit Slip (If Payment made through Bank) Screenshot of Confirmation (If payment made through Paypal).
